2. Data are collected to provide, at your explicit request, information about Halluxmed’s services, to register you for an appointment at Halluxmed or insofar as necessary to provide medical services. Data are processed on the basis of Article 6(1)(b) GDPR.
3. The processed data include ordinary personal data and, where necessary for the purpose described in point 2, data concerning your health.
4. Your personal data may be disclosed to providers delivering medical services jointly with Halluxmed.
5. Data will be processed for the entire period necessary to achieve the purposes described in point 2.
6. You have the right to request from the controller access to, rectification or deletion of your personal data, or restriction of processing, as well as the right to object to processing and the right to data portability under the terms set out in the GDPR.
7. You have the right to withdraw your consent at any time, without affecting the lawfulness of processing carried out on the basis of consent before its withdrawal, insofar as the data are processed on the basis of consent. Consent can be withdrawn by emailing info@halluxmed.pl.
8. You have the right to lodge a complaint with the supervisory authority.
9. Providing data is voluntary, but necessary to fulfil the purpose stated in your email: providing, at your explicit request, information about Halluxmed’s services, registering you for an appointment at Halluxmed or providing medical services.
